Our testers had no pressure buildup when lying on their back—the ideal sleeping position when living with shoulder pain. They slightly sunk into the mattress’s Euro pillow top, and the innerspring coil layer supported their back. In fact, the center is reinforced stronger than the rest of the mattress, specifically targeting the lower back. This allows for better spinal alignment, which also helps the neck position (with the right pillow for neck pain) and distributes force away from prominent joints, like the shoulder.
We like that the mattress is bouncy while also isolating motion. Sleepers have the perk of a mattress that supports their own movement without being disturbed by their partner’s movements. Because the mattress was so firm, our average-weight testers felt like they were lying close to the surface and not comfortably sinking into it. This mattress is likely too firm for light-to-average-weight people. The Titan Plus may work for some average-weight back sleepers who prefer a firmer mattress, but our average-weight tester didn’t feel completely supported when lying on their back or side. Our Reviews Team has dedicated more than 1,000 hours to researching the mattress industry, consulting at least 10 health care professionals, and testing roughly 40 brands and 130 unique mattress models for quality and comfort. We’ve also interviewed 16 older adults who recently purchased a mattress and surveyed more than 3,200 recent mattress customers to understand their experiences shopping for a new bed.
Each mattress undergoes a rigorous 36-page testing procedure to score its different characteristics, including firmness, support, and other aspects most shoppers consider. Like most mattress brands, we use a one-to-10 scoring method when testing firmness, with 6.5 being the true medium-firm. Our testers rate this by lying on the surface and comparing the feel to other mattresses we’ve tested. Material density and coil strength influence firmness the most.

How to choose a mattress when you have shoulder pain
When looking for a mattress to ease shoulder pain, key considerations are pressure relief, functional support, and the right firmness. But they aren’t the only things to keep in mind. The best mattress for shoulder pain will also address other needs and preferences to ensure a balance of comfort and support. Here’s what to think about as you shop.
If you’re experiencing shoulder pain, discuss your symptoms with a doctor and ask if a physical therapy referral is appropriate. You’ll work with your doctor and physical therapist to find long-term treatment options.
Sleep position
The most appropriate mattress construction often depends on the sleeper’s position. Side sleepers benefit from a mattress that’s soft enough to conform comfortably around the shoulders and hips without sacrificing adequate support to keep the spine aligned. Back sleepers need a slightly firmer option to support the lower back and prevent sag. Stomach sleeping may feel difficult for those with a shoulder injury because it’s difficult to move in and out of the position without provoking pain.

Body type

Innerspring
Innerspring mattresses combine a generous layer of coils, which provide a supportive, bouncy, and responsive feel, with a thinner layer of comfort materials on top. Coils help promote airflow as well, which is why innerspring beds don’t often retain heat the way some other mattress types do.
Many innerspring mattresses are in the medium-to-firm range and don’t often isolate motion well. Side sleepers with shoulder pain should consider innerspring mattresses carefully. While they may work well on a case-by-case basis, they can lack the contouring and cushioning sensitive joints need for a good night’s sleep.
Foam
Foam mattresses are made with layers of memory or poly foam (or both), which can vary in density and structure. Softer foams contour closely around the body’s curves to relieve pressure, while denser foams offer more support and a firmer feel. That can make foam mattresses a good fit for people with shoulder pain, assuming they like the sunken sensation that’s common with this material.
Foam mattresses are excellent at dampening motion, so they work well for couples. But, they’re also known for trapping heat, which can make them problematic for hot sleepers.
Hybrid
Hybrid mattresses combine foam and springs to deliver both pressure relief and responsive support. They work well for a wide range of sleepers and sleep styles, and you can find options that contour closely, minimize motion transfer and heat retention, and respond well.

Latex
Eco-friendly brands and models often use latex. It’s a natural and eco-friendly alternative to foam with comparable pressure relief. But it’s more responsive and resilient and doesn’t trap heat in the same way. It also has a gentle buoyant feeling, which really appeals to some sleepers.
For people with shoulder pain, a latex mattress can be a great option, but it does add weight to a mattress. We recommend investing in white-glove delivery if you purchase a latex bed. It’s also more expensive because it’s harder to source.
